CASE STUDY SUBMISSION TIME IS OVER!
All participants are welcome to submit case studies according to workshop topics:
- Rational geriatric prescribing – home care and nursing home care patients
- Medication review in hospital setting
- Medication review in primary care setting
- Pharmaceutical policy and education about medication review
For case studies please use the following structure:
* Title
* Case description and details (description of the medication history, medicines (both POM and OTC medicines) used and patient details).
* Recommendations after medication review, information to be provided to patient and to health care specialist.
All case studies will be evaluated by workshop leaders. Accepted case studies will be discussed during workshops or will be available on the conference website.
The case should be saved as a document or PDF file and sent to daisy.volmer@ut.ee (case study submission time is over!)
You will be sent a confirmation mail upon receipt of the case file. If you don’t receive a confirmation within a reasonable time, please contact us.
